Lost and Stolen Passports, Visas, and Arrival/Departure Records (Form I-94)
To prove their country of citizenship and legal status in the United States, foreign nationals entering the country must have valid passports, visas, and Arrival/Departure Records (paper Form I-94 or admittance stamp).
As soon as possible after arriving in the country, visitors are encouraged to make copies of their passport biography page, U.S. visa, entry stamp, and printed Form I-94, if applicable.
You may stay in the country for the length of your permitted stay, which is shown on your entrance stamp or document Form I-94, Arrival/Departure Record if you are a foreign national visiting the country temporarily and you misplace your U.S. visa.
To leave the United States and enter another country, you will require a valid passport. If the document Form I-94 is lost or stolen, you need to replace it right away. You have to take the following a few steps:
Police Report
Report any missing or stolen documents at the nearby police station. You will have to submit copies of the original documents if they are accessible.
A police report that indicates the occurrence will be given to you. Remember to print an additional copy of the report to your files.
Request Replacement of a Lost/Stolen Arrival/Departure Record (Form I-94)
It is the duty of the Department of Homeland Security (DHS) to replace a lost or stolen Form I-94. See the websites of DHS, U.S. Citizenship and Immigration Services (USCIS), and Customs and Border Protection (CBP) for the Application for Replacement/Initial Nonimmigrant Arrival-Departure Document and Arrival-Departure Record, respectively, to file for a replacement I-94.
Report Lost/Stolen to Your Embassy
For information about replacing a lost or stolen passport, get in touch with the nearest embassy or consular office for your residence country. All countries include contact information on their websites.
Report your Visa Lost/Stolen to the U.S. Embassy or Consulate Abroad
Send an email to the Consular Office of the American Embassy or Consulate abroad that granted your visa if it has been lost or stolen.
To get the email address and other contact details, visit the website of the U.S. Embassy or Consulate. Make sure to provide your complete name, birthdate, place of birth, US address, and, if applicable, email address.
Indicate clearly if the visa was taken or misplaced. Scan and send the passport or visa copy, if you have one, to the embassy or consulate. If not, report the type of visa and the passport number associated with the misplaced or stolen visa, if available.
Should instance that you discover your missing visa after reporting it as lost or stolen to the American Embassy or Consulate, your visa will no longer be acceptable for entry into the country.
You must therefore apply for a new visa at the American embassy or consulate in that circumstance.
Applying for a Replacement U.S. Visa
U.S. visas that are stolen or lost cannot be replaced within the country. You have to apply in person at a U.S. Embassy or Consulate abroad to replace a visa.
You will need to submit a written statement proving the loss of both your passport and visa when asking for a replacement. Take a copy of the police report with you.
